If using the self-book function on the right please indicate in the comments section if you wish an in person appointment.

If we have scheduled you for an in person appointment, you must answer NO to the following questions:

Are you experiencing COVID symptoms (cough, shortness of breath, fever, sore throat, nausea or diarrhea)?
Are you waiting for a COVID test result?
Have you been in contact with someone who has tested positive for Covid within the last 14 days, without wearing the suitable protective equipment?.
